This is my immunization record. This record is using US standart for teenager. The yellow side is a announcement that I have not yet taken the dose so I have to take the dose quickly before my departure to US. And this immunization is the main rule for being allowed to register as US student on YES Programee. Ok, from now I have to make a good relationship with needle and make it becomes my best friend. LOL!
